Post Hospital Support
Any in-home supports that you may need after you’ve been ill or in hospital. This may include nursing support, assistance with household tasks or gardening and assistance with community access.
Post Hospital supports can be funded under one of the following categories your NDIS plan depending on the supports you choose:
Core Supports – Assistance with Daily Life
Core Supports – Assistance with Social & Community Participation
Capacity Building Supports – Improved Daily Living
Eligibility for funding is assessed by the NDIS on a case-by-case basis and will be included in your plan if it is considered a reasonable and necessary support that meets your needs and will help you achieve your goals and is also dependent on your situation, plan objectives and aspirations.
